Privacy Policy

Welcome to Ardu LLC’s website. When you visit and navigate our sites and when you communicate with us via our sites, we will not collect personal information about you unless you provide us that information voluntarily. Any nonpublic personal information that you may provide via our sites will be used solely for the purpose stated on the page where it is collected. Where required by law or regulation, you will be able to update any information that you have provided to us by sending us an e-mail. Please refer to the specific pages where data is collected for more information. Ardu LLC will not sell, license, transmit or disclose this information outside of Ardu LLC and its affiliated companies unless (a) expressly authorized by you, (b) necessary to enable Ardu LLC contractors or agents to perform certain functions for us, or (c) required or permitted by law. What is Health Information

Health information is a part of the personal information that Ardu Recovery Center collects about patients. It’s divided into two categories:

“Protected Health Information” is individually identifiable health information, which is created or received by or on behalf of a covered entity (for example, a healthcare provider or health plan). It relates to an individual’s past, present, or future physical or mental health condition; their provision of health care; and their past, present, or future payment for their provision of health care. Protected Health Information is defined by the federal Health Insurance Portability and Accountability Act of 1996 (HIPAA), and is subject to the protections of HIPAA.

“Additional Health Information” is a subset of health information that does not fall within the category of Protected Health Information. For instance, it encompasses an individual’s information related to past, present, or future physical or mental health conditions; their provision of health care; and the past, present, or future payment for their provision of health care. However, the difference is that such information was not created or received by us from or on behalf of a covered entity. We may collect Additional Health Information in connection with our site or applications.

How We Use and/or Share Protected Health Information and Additional Health Information

We may use and/or disclose Protected Health Information and Additional Health Information to provide the services as described in our policy. Our use and disclosure of Protected Health Information is limited by the main federal health privacy law known as the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), and the Notice of Privacy Practices available from your provider.

We will use and disclose Protected Health Information only to provide services to the patient or the provider, for those uses and disclosures permitted by HIPAA, and the agreement that we have with your provider. In the event that there is a conflict or inconsistency about the handling of Protected Health Information between (i) this Privacy Policy and (ii) our compliance obligations with HIPAA and contractual commitments with providers, then the latter will govern.

My Personal Information

You may also contact us directly if you would like to review, correct, update, delete, or limit our use of your personal information by sending us a message here. In your request, please let us know what information you would like us to modify, whether you want your personal information deleted from our database, or if you have any limitations regarding some of the data in your personal information. We will comply with your request as soon as possible. 

Please note that in order to comply with certain requests to limit our use of your personal information, we may need to terminate your account and ability to access and use our services. You will need to agree that we are not liable for such termination. Although we will use reasonable efforts, you understand that it may not be technologically possible to erase every record of your personal information from our systems. The need to back up our systems to protect data from inadvertent loss means that a copy of your personal information may exist in a non-erasable form that may be impossible for us to locate and erase.

Patient Rights

You have certain rights when it comes to your health information. This section explains your rights and some of our responsibilities to help you.

  • Ask to see or get a copy of your health and claims records and other health information we have about you.
  • We can provide a copy or a summary of your health and claims records.
  • You may ask us to correct your health and claims records if you think they are incorrect or incomplete.

  • You can ask us not to share or use specific health information for treatment, payment, or our operations.
  • We can and may say “no” to your request if it would affect your care.
  • You can ask for a list of the times we’ve shared your health information, who we shared it with, and why.
  • You can ask for a paper copy of this notice at any time.
  • If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information.
  • We will make sure the person has this authority and can act for you before we take any action.

Ardu LLC is committed to ensuring that your information is secure. In order to prevent unauthorized access or disclosure, we have put in place suitable, physical, electronic and manageable procedures to safeguard and secure the information we collect online. Ardu has put together a protective firewall around the information stored at this site. We periodically subject our site to simulated intrusion tests and have developed comprehensive disaster recovery plans.
